what else do i need with this to use in new shower
A. This does not include the Shower Valve Trim or valve. The Valve is the product that actually mixes your hot and cold water behind the wall and then diverts the water into this shower system. The Shower Valve Trim is the decorative cover and handle that you use to turn the water Off / On and Hot / Cold.

How do you turn it on and off?
A. The American Standard 1662.604 is just an outlet and does not include a mixing valve to turn on and off. It would need to be paired with a shower system or pressure balanced valve and trim like the American Standard T430.500 and it's valve.
