Does it have a pause control?

The Delta 57011 does not have a pause feature.

Does it come with supply elbow or tee to put on existing shower arm

The Delta 57011 does not come with a supply elbow or shower arm diverter. Those would have to be purchased separately.

We are pairing this with a dual control shower from the Grail collection. Do we need a diverter valve?

The Delta 57011 would need a diverter to be installed with it, yes. That will allow water to go from the shower head to the handshower.

Is the hose made of metal or plastic?

The Delta 57011 has a plastic hose.
