Is this a two-piece item with a spout & a separate beveled trim ring?

No, the Kohler K-10280-4 is a single piece design diverter spout with rosette integrated with the spout.

How does the Diverter lever function . Do you pull up on it or does it flip.

The Kohler K-10280-4 diverter is pulled up to activate.

What is the difference between K-10281-4 and K-10280-4 ?

The Kohler K-10280-4 is a NPT mount with threads, and the K-10281-4 is the slip fit version for mounting to copper tubing.

Sorry, I just noticed which collections. However, I still can't find the 2 faucets to match the spout and plumbing needed to complete this purchase

The Kohler K-10280-4 can go with the T10277-4. The Forte collection doesn't have a two handle system.
