Will this trim work on a K-671-K volume control valve?

The Kohler K-T12189-4 is compatible with a k-728-k valve.

Can it be left on the handheld so when you turn it on it defaults to the handheld? And can the bar with the handheld be placed to the right of the diverter the picture shows it to the left.

The Kohler K-T12189-4 can be left in the position to default to the handheld. The bar can be placed as desired and applied by your contractor.

Do I need the rough in valve if all I use is one shower head?

The Kohler K-T12189-4 is a diverter handle. It is used when the system has more than one function that needs to be operated. If you are using the diverter handle you will need the rough in valve that goes with it.

WE have an existing Kohler K-T12189-4 Fairfax Single Handle Diverter Trim with Metal Lever Handle and 4 Ports that I would like to replace the trim kit with the Kohler Finial collection. Are these trim kits universal?

Is there a diverter valve with more than four ports?

The Kohler K-T12189-4 is at 3 port - 6 function diverter valve. There is not a four port valve available at this time. However the 3 port allows 6 separate functions.
