Does Moen make a 5" escutheon plate, or anything smaller than 7"?

Moen doesn't make anything smaller than 7", it is their standard.

Does this escutcheon actually have the holes in the 5 & 7 o'clock position as the picture shows?

The Moen 97582 does have the holes at 5 and 7 o'clock.

Moen does not make any type of escutcheon that has 5-7 screw holes.

Is this plate recessed so the back portin goes into the wall - by what distance?

The Escutcheon plate rests on the finished wall and does not go back behind the wall.

The Moen 97583 Part is a polished brass, 5-7 screw escutcheon plate, which also will require use of the Moen 99498 screws and the Moen 99497 stop tube.
