The Progress Lighting P5968 does not include the tiles. You will have to buy them separately. The part number for the tiles are P5970. There are 3 more numbers on the back of that which are the code for the numbers that you need to order.
The Progress Lighting P5968 is a 5 number fixture so if you only have 4 then ordering a blank would be the best option for that.
