Is there a maximum size on the cabinet opening where it is not advised that this will fit properly?

The Rev-A-Shelf 5WB2-1218 per the spec sheet, requires a 11.5 inch wide opening. That is the only stipulation that is made on the document.

Instructions state upper basket can be mounted at two different heights. If the lower height is selected, is the minimum height required still 19.125"?

Yes, you would still need a clearance height of 19 1/8" if you mounted the upper basket slides at the lower installation point. The Rev-A-Shelf 5WB2-1218 is equipped with 2 U-brackets that require that specific clearance.

Does the 5WB2-12" wide series come in a 22" depth or just 18"

The Rev-A-Shelf 5WB2-1218 is an 18" depth model, and the 22" depth model is 5WB2-1222.
